Credit Application

Thank you for applying for credit with FloridaInfusion. Completing our Credit Application is easy.

1. Click the button above.

2. Type in the requested information using your "Tab Key" to move around. Please be sure to complete the application in its entirety.

3. When you have finished, please print out the application.

4. Please check the application for completeness.

5. The application must then be signed by a principal or owner (no facsimile signatures or stamps please).

6. Return the completed and signed application to FloridaInfusion, 1053 Progress Court, Palm Harbor, Florida, 34683, via Overnight Mail and receive a $5.00 credit on your first order.

Once your Credit Application has been received and approved, a friendly FloridaInfusion Customer Service Representative will contact you. Our Customer Service Representative will advise you of your new Account Number, your credit limit and  payment terms, and will be happy to take your first order. Should there by any problems with your Credit Application such as missing or incomplete information, a representative from our Credit Department will contact you.
